I have bad bots visiting my site, and my site gets rejected by Google AdSense, If I make visits from United States go through Captcha in cloudflare, Is this hurting my site? My visitors from Arab countries

Every CDN is capable of Geo blocking and it depends on whether or not you can make the visits that are from countries that you don't serve relevant visits. In other words you may not provide services for the states but you could always provide blog posts full of schema markup on how to's, list type, and so many others to try and get position zero or PPA or FAQ.
If you do this right it won't matter that people from other countries are visiting so long as your monitoring what they're doing and so long as you see that they are generating UX factors. So it is likely you should include traffic from the states but I can't say for sure.
